1. What is the special need of your child?
My child
has speech, reading, and English problems.
2. What are your personal goals for the
education of your child?
For him to keep up with his
class work and to advance to the next grade level.
3. What is your current relationship
with the school, special education, and administration?
I have regular meetings with
counselors and IEP teachers.
4. How can it be approved?
I think
that it would be better to work more one on one.
5. What form of communication is used
and how often? Is this best?
I have meetings twice a year with the teachers and administration. I do not
think that this is best and would prefer to meet more often to help keep him on
better track.
6. How often do you communicate with
school?
I
communicate with the school four times a year, twice for conferences and two IEP
meetings.
7. How effective is the special education program?
I think that the special
education program is very effective it helps keep him on the right track for his
grade level.
8. How flexible is the program to meet the needs of your child?
The program is good,
they change the as he goes for his ability as it increases.
9. What problems have you had with
school?
I do not
have any problems with the school. They are very good about communicating with
me if any changes need to be made.
10.Are you part of any parent group?
No, I am
not a part of any groups.
11. What communication do you have with other parents?
I do not really have much
communication with any of the other parents.
12. What community involvement does your
child have?
He is involved in youth
group and Sunday school.
13. Are you satisfied with your
communication with your child?
At times I am satisfied, but
there is still some improving that could be done.
